All registrations must be submitted online.
No registration is valid without payment.
The deadline for registering with the Early Registration fee is 16 July 2012.
If you are an EACMFS Member you are entitled to pay the reduced fee. Please make sure your EACMFS membership fee payment has been settled.

REGISTRATION FEES

The Full Congress registration includes entrance to the Opening Ceremony and Presidential Reception (space permitting, pre-registration needed), participation in all the scientific sessions (except masterclasses), access to the commercial exhibition, congress material, congress abstracts, coffee breaks, working lunches and a certificate of attendance.

The Accompanying Persons registration includes entrance to the Opening Ceremony and the Presidential Reception (space permitting, pre-registration needed).

TRAINEES MEETING

EACMFS Member residents attending the congress can attend the trainees meeting, held on Monday, 10 September at no additional cost.

For residents not attending the congress, the following registration fees for the trainees meeting apply:

* To qualify for trainee registration fees, applicants must obtain a letter from the Head of the Department to verify their trainee status.

On Monday evening (Sep 10), a Trainee Party is included in the registration for the trainees meeting.

SOCIAL PROGRAMME

LIABILITY
In the case of Government intervention or regulation, military activity, strikes or any other circumstances that make it impossible or inadvisable for the EACMFS XXI Congress to take place at the time and place provided, the participant shall waive any claim for damages or compensation except the amount paid for registration after deduction of actual expenses incurred in connection with the congress and there shall be no future liability on the part of either party.

PAYMENT, CANCELLATION AND REFUND POLICY
All costs referred to are in EUR (€). All payments must be made in EUR (€). All bank transfer costs must be covered by the payee, otherwise you will be charged for them on site. Only Croatian citizens with bank accounts in Croatian banks can make payments in HRK (KN). Payments must be received prior to the congress. Delegates with outstanding payments will be asked for payment on site, proof of payment or a guarantee by credit card. If one of these cannot be supplied, the delegate will be refused permission to attend the congress.

CANCELLATION OF CONGRESS REGISTRATION
In the event of cancellation, written notification should be sent to the PCO Secretariat (c/o 361 Ltd Zagreb).
The following cancellation fees will apply:
From 15 December to 15 May, 2012: Refund minus € 50.00 handling fee
From 16 May to 27 June, 2012: Refund minus € 80.00 handling fee
After 28 June 2012: No refund

For cancellations received before 29 June, refunds will be made minus 10% of amounts paid. After 29 June, no refunds will be allowed.

All refunds will be processed and issued within 60 days after the congress.

CANCELLATION OF MASTERCLASSES, TRAINEES MEETING, SOCIAL PROGRAMME AND OPTIONAL TOURS
In the event of cancellation of masterclasses, trainees meeting, social programme and tours, written notification should be sent to the PCO Secretariat (c/o 361 Ltd Zagreb). For cancellations received before 22 June, refunds will be made minus 10% of amounts paid. After 22 June, no refunds will be allowed.
All refunds will be processed and issued within 60 days after the congress.

PAYMENT
The online registration site is a secured site. Major credit cards such as American Express, VISA and MasterCard are accepted. Payments by wire transfer are accepted until 15 August. Full payment must be received prior to the congress. All bank transfer costs must be covered by the payee, otherwise you will be charged for them on site.

SECURITY
For security reasons, all registrants will be issued a badge onsite which is required to be worn at all times during the congress and at any organised functions. Badge swapping is strictly forbidden.
